FORMS and Documents

Norfolk Collegiate and the Virginia State department of Health requires that all students have current forms and records on file with the school. Below are some of the forms in which parents and students will need to have on file with the school. 

Please note that many of the required forms are now part of the Magnus Health Portal experience and will be completed by you, the parent, in Magnus.  For specific questions about need forms, please contact your respective school division. 

 

Medical Forms

  • Commonwealth of Virginia School Entrance Health Form: State law (Ref. Code of Virginia § 22.1-270) requires that your child is immunized and receives a comprehensive physical examination before entering public kindergarten or elementary school. The parent or guardian completes this page (Part I) of the form. The Medical Provider completes Part II and Part III of the form. The Commonwealth of Virginia School Entrance Health Form should be filled out and accompany the application. Download the Commonwealth of Virginia School Entrance Health Form (PDF)
  • Athletic Physical Form: All students planning to participate in interscholastic athletic competition and cheerleading must submit an Athletic Physical Form (PDF) and parental permission forms each academic year prior to the first organized practice. The forms should be submitted to our athletic trainer, Justin Seligman. Download  Athletic Physical Form (PDF)

 

TRANSPORTATION FORM 

  • Request for Transportation Form: If you would like for your child(ren) to ride the bus to school, please complete and return the Request for Transportation Form and return it to the business office before school begins. Bus routes are determined once the school and bus company (TranQuest) determines routes that are most convenient for all families.
