State law (Ref. Code of Virginia § 22.1-270) requires that your child is immunized and receives a comprehensive physical examination before entering public kindergarten or elementary school.The parent or guardian completes this page (Part I) of the form.The Medical Provider completes Part II and Part III of the form. The Commonwealth of Virginia School Entrance Health Form should be filled out and accompany the application.

Specific Immunization Information for parents of Rising 6th Graders:
In preparation for entry into the Middle School, there are some important immunizations that the Virginia Department of Health requires prior to entering sixth grade.
The Virginia Department of Health (VDH) states that: “Effective July 1, 2006, a booster dose of Tdap (tetanus, diphtheria, pertussis) vaccine is required for all children entering the sixth grade, if at least five years have passed since the last dose of tetanus-containing vaccine.”
This Tdap booster must be administered prior to school entry in August.
As well, the VDH advises: “Effective October 1, 2008, a complete series of 3 doses of HPV vaccine is required for females. The first dose shall be administered before the child enters the 6th grade. After reviewing educational materials approved by the Board of Health, the parent or guardian, at the parent’s or guardian’s sole discretion, may elect for the child not to receive the HPV vaccine.”
Immunizations may be obtained from your child’s physician, military clinics, or the health department. Please provide a complete updated immunization record from your child’s health care provider by August 1. Your child will not be able to attend school if this documentation is not provided.
This documentation is separate from the Sports Physical provided to the Athletic Department.
